Hospital Social Work
What services do we offer?
We offer a social work service within a hospital setting. We can help you and your family with a range of personal and practical matters arising from your stay in hospital:
- Assessment - to build up a picture of the sort of help you need.
- Information - about your rights and choices.
- Support - to assist you if you feel worried or concerned in hospital.
- Help on leaving hospital - working with doctors, nurses and therapists we can assist you with plans for your care when you leave from hospital.
- Counselling - for patients, relatives, carers and others.
- Liaison - with the hospital and other agencies, for example, home care, meals on wheels, housing and benefits.
Where are we?
Hospital Social Work team are based at Glan Clwyd Hospital but also offer a social work support service at Abergele Hospital (Denbighshire residents only), HM Stanley Hospital at St Asaph, Denbigh Infirmary, Llangollen Hospital and Ruthin Community Hospital.

- We will offer you a service in the language of your choice whenever possible.
