Members of Denbighshire's Cabinet have been informed of arrangements for monitoring an action plan drawn up in response to a serious case review following the deaths of two children in Bodelwyddan in November 2006.
The Local Safeguarding Children Board carried out a serious case review to identify whether there were lessons to be learned from the case about the way in which local professionals and agencies work together to safeguard children, to identify clearly how those lessons will be implemented, to improve inter-agency working and to identify examples of good practice.
The main findings concluded that the children's deaths could not have been prevented nor predicted and the children were not at risk of significant harm until immediately before their deaths. However, the review did conclude that there were lessons to be learned by the agencies involved, including the need for children's needs to be included in reviews of parents who have severe mental illness problems and there should have been more contact between those professionals working with adults and those responsible for children.
The action plan is being implemented by the Council, the Conwy and Denbighshire Adult Mental Health Partnership, Denbighshire Local Health Board and North Wales Police.
